The Discharge Planner coordinates healthcare discharge services, focusing on identifying community resources and scheduling follow-up appointments for patients nearing release. They provide both individual and group psychoeducational sessions to support patients’ transitions into the community. The role includes facilitating benefit applications and coordinating medication or residential needs post-release. Discharge Planners maintain contact with community providers and document all activities in health records. The position requires close collaboration with facility and parole staff to ensure continuity of care.
Coordinate discharge planning services, including MAT, medical, and mental health programs
Provide individual and group psychoeducational sessions for patients scheduled for release
Facilitate benefit applications and connect patients to community resources
Arrange post-release appointments, referrals, and residential placements
Document all services in the health record and maintain communication with community agencies
EDUCATION
Graduation from an accredited School of Nursing is required
Bachelor's degree in Social Work, Business Administration, or other health-related fields from an accredited college or university is preferred
EXPERIENCE
Previous experience in developing successful transition plans involving community resources for persons with medical, substance use, and/or mental illness is preferred
LICENSES/CERTIFICATIONS
Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment
Must maintain CPR certification, and any other certifications (such as First Aid) required by contract
