How you make a difference

The Admissions and Discharge Coordinator receives, reviews, and organizes all referrals, associated documents, and communication for admission to and discharge from the hospital.  The Admissions and Discharge Coordinator is responsible for the actual admission and discharge process as well as the ongoing communication with community referral sources up until admission. 



Key Responsibilities

  • Coordinates the admission process by communicating with receiving facilities and obtaining necessary admission information on referrals, and maintains knowledge of hospital criteria and legal aspects of admissions. 
  • Reviews referrals thoroughly, sets up admissions with the Clinical Director, maintains communication with regional districts, and keeps a tracking list of referred patients. 
  • Organizes all admission paperwork, provides comprehensive reporting on admission activity to Administration, welcomes and performs initial admission paperwork for new patients, and sets up new charts. 
  • Demonstrates understanding of risk management, infection control, precautions, fire and disaster procedures, and abuse reporting. 
  • Keeps track of occupancy and vacancies, provides reports to districts on patients in the hospital, and reconciles bed count. 



Qualifications & Requirements

Education

  • High school diploma or equivalent required. 
  • Bachelor’s degree in Business Administration preferred.

Experience

  • Three (3) years of admissions and discharge experience required, two of which must be in a psychiatric setting. 

Licenses/Certifications

  • None required.
